Abstract:
A flexible, substantially non-stretchable, imprintable tape which includes lines of weakness constituting tear lines defining in outline a longitudinal series of symmetrical identification bracelets. Each bracelet has a generally rectangular imprintable portion, a first elongated strap portion at one end of the imprintable portion, and a second elongated strap portion at an opposite end of the imprintable portion. The imprintable portion has a width equal to the width of the tape and the first and second portions each have a width substantially less than that of the tape. The first strap portion has a longitudinal edge aligned with one of the side edges of the tape and an opposite longitudinal edge defined by one of the lines of weakness. The second strap portion has a longitudinal edge aligned with an opposite side edge of the tape relative to the first strap portion and an opposite longitudinal edge defined by another one of the lines of weakness. No part of the tape is wasted.

Abstract:
A plurality of identification bracelets removably attached to one another, each bracelet having a first end which includes a cover tab and a second end defining a closure tab having an adhesive portion. The cover tab of a second bracelet removably overlies the adhesive portion of a closure tab of an adjacent first identification bracelet. Upon separating the first and second bracelets, the adhesive portion of the first bracelet is exposed and the cover tab remains with the second bracelet as it is removed. The adhesive portion of the closure tab of the first bracelet, is adhered to a surface of the identification bracelet as it is moved from an open position to a closed position encircling an object to be identified. The bracelets may be connected end-to-end to form an elongated strip or along elongated side edges to form a sheet of identification bracelets.

Abstract:
A closure mechanism receiving aperture presents a substantially contiguous, uninterrupted planar surface on a flexible medium, primarily for use with identification devices, i. e. , wristbands. The aperture does not interrupt the printable area so that printer ink may be printed over the aperture without passing through. The inventive aperture also does not produce waster material or chads which can jam or foul-up a printer. An inventive wristband includes both snap closure and adhesive closure mechanisms that can be used at the same time.

A multi-part form includes a wristband with related tags and labels. The tags are configured for mounting on the wristband after the wristband has been secured to a person of object to be identified. The multi-part form is also configured with a wristband portion separate from a plurality of utility groups, each of which is separate from the other, by a street devoid of media layer. The layers that comprise the multi-part form are preferably solvent resistant so as to make the identification products more durable and longer lasting.
